You may request a copy of your health records and other information maintained by Weigh To Go.

You may request restrictions on certain uses or disclosures of your PHI.

Right to Confidential Communications

You may request that we communicate with you through alternative means or at alternative locations.

Right to an Accounting of Disclosures

You may request a list of certain disclosures of your PHI made by Weigh To Go.

Right to a Paper Copy

You may obtain a paper copy of this Notice at any time, even if you have agreed to receive it electronically.

OUR RESPONSIBILITIES

Weigh To Go is required by law to:

  • Maintain the privacy and security of your PHI;
  • Provide you with this Notice of Privacy Practices;
  • Notify you following a breach of unsecured PHI when required by law;
  • Abide by the terms of the Notice currently in effect.
ELECTRONIC COMMUNICATIONS

If you communicate with us through email, text message, mobile applications, telehealth platforms, or our website, we will take reasonable measures to protect your information. However, no electronic communication system can be guaranteed to be completely secure.

BUSINESS ASSOCIATES

We may share PHI with third-party service providers that perform functions on our behalf. These providers are required by law and contract to safeguard your PHI and comply with applicable HIPAA requirements.

CHANGES TO THIS NOTICE

We reserve the right to change this Notice at any time. Any revised Notice will apply to all PHI maintained by Weigh To Go and will be made available upon request and through our website.
